Wales fly-half Sam Costelow will start and Blair Murray switches to full-back as Warren Gatland makes four personnel changes to face world champions South Africa in Cardiff on Saturday.
Costelow replaces Gareth Anscombe, while Murray moves from wing to full-back with Rio Dyer returning and Cameron Winnett dropping out of the squad.
In the forwards, Exeter lock Christ Tshiunza replaces the injured Adam Beard, while number eight Taine Plumtree is recalled for Aaron Wainwright.
Beard's absence is the only injury enforced change.
Gloucester lock Freddie Thomas is set to win his first cap from the replacements' bench, which includes wing Josh Hathaway, but Anscombe is not included in the squad.
It will be the first time New Zealand-born Murray has started a professional game at full-back.
Murray arrived at the Scarlets for the beginning of the 2024-25 season and has made six starts on the wing.
